Health Portal

For Parents/Guardians registering and completing health forms for the portal:

• choose a user name with no spaces
• enter the parent preferred email on file with Phillips Academy 
• enter parent name
• if you have previously registered and forgot username or password or both, click on "Forgot Password" and you'll receive an email with the information

For Students registering for Opt In to Texting, Online Scheduling, etc:

• choose a user name with no spaces
• enter your Phillips Academy email address
• enter your full name on file with registrar's office name
• if you have previously registered and forgot username or password or both, click on "Forgot Password" and you'll receive an email with the information

After clicking on the submit button, an email confirming your registration will be sent to the address on file and you will be able to choose a password.
